This Home Health Physical Therapist opportunity is based in
Beckley, West Virginia, a welcoming community that serves as a hub
for Southern West Virginia. Beckley offers the ease and

patients.In this role, you will provide skilled Physical Therapy
services to patients in their homes across Beckley and nearby
communities. You will manage a caseload primarily consisting of
adult and geriatric patients with a variety of diagnoses, including
post-surgical, orthopedic, neurological, cardiopulmonary, and
general deconditioning. You will conduct comprehensive evaluations,
develop personalized treatment plans, and implement interventions
designed to improve functional mobility, safety, strength, balance,
and overall quality of life in the home environment.A typical day
includes traveling between patient homes, performing initial and
follow-up assessments, delivering therapeutic interventions, and
educating patients and caregivers on home exercise programs, fall
prevention strategies, and safe use of assistive devices. You will
complete home safety evaluations and make recommendations for
environmental modifications and equipment to support independence.
Patient volume and visit schedules are structured to balance
productivity with quality, allowing you to spend meaningful time
with each patient while maintaining timely documentation in the
agencys EMR system.You will work as part of a supportive
interdisciplinary team that may include nurses, occupational
therapists, speech-language pathologists, social workers, and home
health aides. Collaboration is emphasized through regular
communication, care conferences, and coordinated treatment planning
to ensure patients receive comprehensive, well-rounded care. While
you will operate independently in the field, the team environment
and leadership structure are designed to provide guidance, clinical
support, and opportunities for ongoing professional growth.Home
