promote continuity of care, optimal patient outcomes, patient
satisfaction, cost efficiency, and compliance. Reports to: Director
of Quality FLSA: Non-exempt Education: Graduate of a Program in
Discipline Required; Graduate from an accredited school of nursing
BSN Preferred Licenses: RN licensure to practice to work in the
state of Kentucky; Obtain ACM (Accredited Case Manager)
certification within 2 years of hir/transfere date. *New Graduates
may work under a KY provisional license for up to 6 months with the
expectation that an RN license will be obtained within the first 6
months of employment. If an RN license is not earned within 6
months of employment, the provisional licensee must immediately
cease practicing in a licensed nursing position. Certifications:
Basic Life Support (BLS) - to be obtained within 30 days Required
Skills Requires critical thinking skills, decisive judgment and the
ability to work with minimal supervision. Must be able to work in a
stressful environment and take appropriate action. Essential
Functions Consults with nursing staff and multidisciplinary team
regularly to evaluate patient's status and appropriateness of
medical care, including admission, length of stay, transfer and
discharge. Monitors patient and family satisfaction. Responds to
questions and complaints from patients, family members, and payors
regarding care. Participates in discharge planning including
coordinating patient transfers to other facilities and coordinating
community resources. Provides discharge education and resource
referrals to patients. Performs chart review to identify actual or
potential issues with service delivery, patient outcomes and
satisfaction, compliance, cost, and reimbursement. Non-Essential
