Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 5x8-Hour (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience, depending on the specialty License: Must have a valid nursing license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase (HCHB) and OASIS Reliable transportation and current driver's license Agency-required liability insurance Comfortable with home health assignments requiring travel to patients' homes Willingness to accept patients regardless of distance from previous appointments Understanding of documentation requirements and productivity expectations Description: Provides coordination and direction for the delivery of home nursing care according to physician's orders. Applies the nursing process competently and in line with agency policies and procedures. Reports to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Conducts home visits and participates in periodic office meetings. Must travel up to 75 miles from the home clinic for patient visits. Mileage is reimbursed at the IRS standard rate. Contractors will be assigned between 5-10 patients per day. Productivity expectation is a minimum of 30 points per week. Each type of visit and activity has a different point value. Types of cases include Start of Care, Wound Care, and Follow-up Visits. On-call duties may be required one weekend a month. All documentation must be completed within 24 hours of the appointment. No guaranteed hours; flexibility in scheduling and patient acceptance is required. Local candidates are accepted at the same rate. Experience in home health or hospice is preferred.
