Pay Information $2,026 per week About The Position ID: 54913117 Shift: Day 5x8-Hour (08:00 - 16:30) Description: JOB SUMMARY: Responsible for facilitating and coordinating the Prospective Payment System (PPS) for the Inpatient Rehabilitation Unit utilizing the IRF-PAI. Responsible for coordinating all Minimum Data Set (MDS) assessments and plan of cares for the Swing Bed Unit. Shift dates/schedule: DayWeekends Required: NoCall Required: NoFloating required: NoCertifications required: WV RN/ BLS of years of experience required: 3-5Skills preferred: RN with Resident Assessment Coordinator Certification (RAC-CT)Skills required: MDS Completion/: Associates degree in nursing Current West Virginia state licensure as a registered nurse Two years of direct clinical experience required with prior rehabilitation experience preferred. CPR certificationPreferred: Bachelor s or master s degree in nursingESSENTIAL JOB FUNCTIONS AND the MDS schedule and maintains MDS' s to meet federal and state requirements.2.Coordinate all MDS activities and assessments on each resident in a timely manner.3.Coordinates all patient care conferences.4.Coordinates the completion of all individual care plans based on Care Area Assessment (CAA) Triggers. 5.Coordinates the completion of all CAA documentation.6.Reviews and interprets the case mix reports and submits to administration as indicated.7.Directs the staff development and training of all MDS related documentation and activities8.Monitors all quality indicators to assess departmental needs and develops and implements programs that are deemed necessary. 9.Responsible for completion and submission of the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI) to ensure accurate payment and to avoid payment penalties.10.Oversees/completes and analyzes and Quality Indicator (QI) data ensuring the accuracy and the completion of the IRF-PAI according to CMS guidelines and prior to electronic transmission.11.Review documentation using the medical record and eRehabData System to support the accuracy and timeliness of the functional scores and comorbid conditions, according to CMS guidelines. 12.Ensures that the IRF-PAI is consistent with the medical record documentation and reflects the functional status and 24-hour care needs of the patient.13.Attends the team conferences to comprehensively assess the data gathered for the IRF-PAI and assists in the development of the Individualized Plan of Care.14.Collaborates with physicians and interdisciplinary team members to ensure the most accurate assessment of the needs and strengths of patients are captured in the eRehabData System and in the medical record.15.Provides physician and staff education to assure accurate and timely completion of the IRF-PAI and to improve Quality Indicator (QI) score accuracy at admission, throughout stay, and upon discharge. 16.Review current facility clinical documentation in the medical record and eRehabData to ensure accuracy to support the correct Case Mix Group (CMG) assignments.17.Identifies target reference dates for the completion of the IRF-PAI for all patients.18.Maintains current knowledge of the eRehabData System and CMS IRF-PAI software and provides expertise on the software within the inpatient rehab setting and contributes to facility process improvement plans.19.Responsible for maintaining continued education regarding Medicare regulations for the Inpatient Rehabilitation Facility and Swing Bed Unit.20.Runs reports from the rehabilitation software to assist with data analytics.21.Participates in performance improvement activities and assists in the development of quality monitors and collection of data.22.Assists with the review/ revisions of policies and procedures on an as needed basis.23.Direct the workflow of Activities Coordinator.24.Attends all meetings, committees, in-services or training sessions, pertinent to perform the position duties.25.Demonstrates effective critical thinking skills.26.Demonstrates excellence in customer service skills.27.Fosters an atmosphere of teamwork and collaboration.28.Performs all other duties that may be assigned.
