Job Description JOB DESCRIPTION – CHW Program Nurse Coordinator
POSITION SUMMARY : Responsible for assisting the Division Director
and collaborating with the ‘ CHW Workforce and Professional
Development Coordinator ’ for the CHW Program. The individual will
be responsible for support of new and existing sites using the
Marshall copyrighted CHW model. The individual in this position
will also have responsibility for quarterly data analysis, site
visits, and providing technical assistance to support and
strengthen the Program. DUTIES / POSITION ACCOUNTABILITIES

Meets requirements for mandatory in-services and continuing
education. ORGANIZATIONAL SUPPORT RESPONSIBILITIES : Attendance –
maintains good attendance in coordination with University policy.
Punctuality – remote work requires availability Monday-Friday from
8a-4p unless employee has approved vacation, holiday, or sick leave
scheduled. Uses time sheet to record time correctly for each pay
period. Follows appropriate processes for requesting accrued time
off and planning for coverage of work responsibilities while off.
WORK ROLE RESPONSIBILITIES : Participates in site visits,
healthcare center, and community meetings/events to increase
project visibility, community awareness of the Marshall CHW
Program. Schedules and organize meetings/events and maintains
agendas, including participation in planning and execution of
monthly learning collaborative and the bi-annual in person CHW
conferences. Maintains updated documentation and records of all
meetings and events related to the CHW Program. Submits accurate
and prompt data reports to review with the Division Director.
Assists the Division Director in implementation, execution, and
completion of grant goals, outcomes, and activities. Collaborates
closely with the Workforce and Professional Development Coordinator
to support and strengthen the CWH workforce. Performs other duties
as assigned or requested. DEPARTMENT / DIVISION SPECIFIC
Responsible for carrying out activities to meet Program goals.
Responsible for coordination of the Program with collaborating
agencies, including payers. Responsible for oversight, technical
assistance, and data collection of community health workers
Responsible for coordinating activities to support Program
evaluation. Responsible for implementing a communication plan and
strategic plan for Program sustainability and growth. PHYSICAL :
Able to travel for site visits OTHER REQUIRED COURSE / TRAINING IN:
Must have advanced knowledge and use of Microsoft Excel Must be
proficient in Microsoft Word, PowerPoint, and Outlook. Must possess
excellent time management and organizational skills Must be detail
oriented with results-driven approach Must be able to perform
descriptive data analysis. Must be skilled at developing rapport
with partners and stakeholders from other organizations and
maintaining working relationships that support the implementation
of the Program. Must have excellent verbal and written
communication skills as well as demonstrated skills in public
speaking, leadership, and group facilitation. Must demonstrate a
clear understanding of professional boundaries and confidentiality
when working with individuals and the professional staff in partner
agencies. Must have a valid driver’s license and reliable
transportation.
